A United States Nuclear Regulatory Commission study has identified and categorized 64 human performance issues related to the integration of new technology in nuclear power plants. One of the recurrent themes spanning many of the identified issues is that of complexity. While plant designers and licensees are seeking greater simplicity, several factors such as increased sensing capabilities, information processing support, automation, and software mediated interfaces are likely to increase operational complexity. The report reference is NUREG/CR-6947.

Spatial disorientation remains a persistent, and significant factor in 15% of fatal aviation accidents. Humans maintain orientation and posture through a system of senses adapted to work on the earth. However, the absence and/or conflicts in sense orientation that can occur in flight can lead to pilot misinterpretation of actual orientation. The Disorient Express in a recent issue of Air & Space provides an excellent summary of the types of disorientation and how they occur.

Papers are being solicited for the Control Room Operations session of the 2009 Canadian Nuclear Society conference scheduled for May 31 to June 03 in Calgary. The objective of this conference session is to provide a Canadian forum for discussion of topics concerning issues and improvements in control room operations. The prevalence and patterns of pilot error in commercial airline accidents does not change with pilot age. Researchers at Johns Hopkins University examined 20 years of commercial airline accidents, and suggest the lack of association between age and error may be due to the rigorous industry selection and certification standards, and frequent competency evaluations. A summary of the study findings can be found in the 2006 July issue of Aviation, Space, and Environmental Medicine.

The Health and Safety Executive in Great Britain has created a human factors toolkit to support the consideration of human factors in facility inspections and accident investigations. The toolkit consists of a series of guides and checklists covering the examination of such topics as competence assurance, human failure, emergency response, maintenance error, communications, and organizational change.

2008 May 23
Hold Short for Runway Safety is a new website created to raise awareness of the measures pilots can take to ensure safety during ground operations. The site was created by the Air Line Pilots Association (ALPA) with the goal to reduce runway incursions, excursions, and confusion. Reducing runway incursion incidents has been a longstanding safety priority with the National Transportation Safety Board.
